Medi-Card Could Save Your Life
Now you can carry your complete medical history, including any special instructions regarding allergies or medications, with you at all times on a plastic card you carry in your purse or billfold.
In case of an accident or other emergency, when seconds count, doctors or emergency room personnel only have to grab this card to get your complete medical history. It has been reduced to microfilm and sealed inside a plastic card which travels with you wherever you go to help answer such questions as - Blood type? Diabetic? Heart condition? Allergic to penicillin or other drugs? Eye glass prescription? Family physician and address? Insurance company and policy number? Shot record? Whom to contact for surgical authorization?
You fill out a standard medical record form which is then returned to the company and reduced to microfilm and sealed in plastic, along with your name, address, social security number and phone number. Cost is $3.95 per card.
